Technical Assistance:
**Discussed with the director, that for degrees that are earned in country's other than the United States, they will need to be translated (if necessary) and then qualified to determine if those studies meet the degree requirements of degrees obtained in the U.S.

Comments:
An unannounced monitoring inspection was conducted today from 11:38am to 1:30pm. There were 93 (infants-5yrs.) directly supervised by 16 staff. The physical plant, 6 staff records, 5 children?s records, medications, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed preparing for and eating lunch, and preparing for and taking a nap. Diapering and hand-washing procedures were also observed. There was an abundant supply of books, toys, and materials for the children. The center was clean and organized. Areas of non-compliance are identified in this report. Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 10 calendar days from today. Description: Based on review, the facility failed to ensure that children's records were maintained with the required information. Evidence: Reviewed 5 children's records and found that CH2 and CH4 did not have documentation of the first day of attendance.

Plan of Correction: Added dates according to Attendance records 5/25/16 Director will follow up 5/25/16

Standard #: 22VAC40-185-70-A
Description: Based on review, the facility failed to ensure that staff records were maintained with the required, minimum information. Evidence: 1) Reviewed 6 staff records and found that Staff 2 did not have documentation of reference checks being completed prior to employment 2) Reviewed 6 staff records and found that Staff 3 was missing documentation of who completed the second reference check and when.

Plan of Correction: Checking each new Enrollment packet is filled in 5/25/16 Going forward Will be sure to keep ref 5/25/16 Checks back in file Director will remember to sign

Description: Based on review, the facility failed to ensure that they maintained a written record of children's serious and minor injuries. Evidence: Reviewed 5 injury reports and found that 1 did not have documentation of the time parents were notified of the injuries.
