Standard #: 22VAC40-185-270-A
Description: Based on self reported incident by the center director, a child was stuck between the loft and the wall in the shooting stars classroom. Evidence: The center director reported that a child was pulling himself up on the loft side to see what the children on top were doing. His head got wedged between the wall and the side of the loft. He called for help and was assisted by the center staff and emergency personnel to ensure he was not injured while freeing himself. The opening space measures greater than 5 inches (5 1/4 inches) but less than 9 inches which is considered a head entrapment hazard. Based on observation, two areas of the center are not being maintained in a clean and safe manner. Evidence: 1. The window frame in the Grasshopper classroom is peeling paint in a small area. 2. The reflective mirror in the gym area has cracks that are spreading past the adhesive tape.

Plan of Correction: Apparatus is closed until gap is remedied. Awaiting report from County Child Care office to schedule repair. Work to be conducted within five business days of report. 1. Window frame scraped and sanded 5/4/2016. Repainted 6/11/2016 2. Reflective coating mended. 5/13/2016

Description: Based on observation, the water temperature in the upstairs bathroom exceeds the maximum temperature of 120 degrees. Evidence: The water temperature was measured on the day of the inspection and measured 130.7 degrees.

Plan of Correction: Thermostat adjusted and preferred hashmark/setting highlighted.

Description: Based on record review, a random sample of injury forms were missing information. Evidence: 15 injury reports were reviewed: 2 were missing the time parent was notified, 2 were missing "future action to prevent reoccurrence", 2 were missing who notified the parent (use staff member's name) and 1 was missing the date.

Plan of Correction: Staff retrained.
