Standard #: 22VAC40-185-190-A Description: Based on interview and record review, the program director does not meet one of the educational requirements. Evidence: The program director has some college. Only one of the classes the director has completed is in a child related field. Standard #: 22VAC40-185-210-A Description: Based on record review, staff 3 does not meet lead teacher qualifications and was left alone with and in charge of children. Evidence: Staff 3 has no college or certifications in a child related field. Staff 3 has only received 20 hours on training with none of those in playground safety or child development. Plan of Correction: The provider's responses for the "plan of Correction" were not received as of 6/2/16 and will not appear on this Violation Notice.
Standard #: 22VAC40-185-530-A Description: Based on interview, the administration was unable to provide documentation of at least one staff member present who has a current certificate in first aid. Evidence: The administration was advised to provide documentation of at least one staff member that has a current certification in first aid within the last three years by 5/13/16. As of 5/24/16 no documentation was provided. Plan of Correction: The provider's responses for the "plan of Correction" were not received as of 6/2/16 and will not appear on this Violation Notice.
