Description: Based on observation, the facility did not ensure that areas and equipment of the center, inside and outside, shall be maintained in a clean, safe and operable condition. Evidence: 1. There was a small break on the underside of 2 swings which exposed rusted metal. 2. Management staff confirmed this.

Plan of Correction: The director removed the swings during the inspection.

Description: Based on measurements, the facility did not ensure that resilient surfacing shall comply with minimum safety standards forthe area underneath and surrounding equipment that requires a resilient surface. Evidence: 1. On one side of a climbing dome, the resilient surfacing was 4 inches deep, ending on a hard surface, at a distance of 4 ft. 6 inches from the perimeter of the dome. 2. Management confirmed this.

Plan of Correction: The climbing dome was removed from playground during the inspection.
