22VAC40-185 SPECIAL SERVICES.
The licensing inspector initiated an unannounced inspection in the center on May 16, 2016 again on May 26, 2016 and it was completed on June 13, 2016 in response to a self reported incident received by the licensing office regarding staffing and supervision and special services. Interviews with staff members and parties involved were conducted. Based on observations and a review of center documentation available during the inspection, a preponderance of evidence does not support the allegation regarding special services however there was a preponderance of evidence regarding staffing and supervision and therefore the complaint is determined to be valid. Areas of non-compliance are noted on the violation notice. The results of this inspection were reviewed with the center director during the exit interview. Standard #: 22VAC40-185-340-A Description: Based on a review of documentation and staff interviews, it was determined that on Thursday, May 12, 2016 staff members did not fully ensure one chid's care, protection, and guidance. Evidence: During the inspection, it was confirmed that on May 12. 2016 at approximately 9:05 a.m. staff member #1 fed child #1 a bottle of breast milk that was intended for use only by child #2. Staff #1 said "the bottle was in his (child #1) mouth for only 2 to 3 seconds and and then I immediately realized it". The bottle was intended for child #2 and not child #1. Center staff confirmed the bottle was dumped out and that "the bottle was practically full" said staff #2. Plan of Correction: 22VAC40-185-(5)-340-A All caregivers have been re-trained on proper feeding procedures effective 5/26/16. All caregivers will verbally confirm a child's identity and the correctly labeled bottle with another tenured/veteran teacher effective 5/26/16. Management will observe daily to ensure compliance. .
