An unannounced monitoring inspection was conducted on June 14, 2016 from approximately 7:00 AM to 9:15 AM. Three staff were present with twelve children. Children were observed participating in table activities. Licensing Inspector reviewed standards related to supervision, activities, equipment, supplies, emergency evacuation and shelter-in-place drill documentation, fire and health inspection reports, injury reports, liability insurance coverage documentation, children & staff records, emergency supplies and required postings. Violations were reviewed with and verified by ____ during the exit interview conducted on this date.
Standard #: 22VAC40-185-60-A Description: Based on review, facility did not ensure 1 of 3 children's record has documentation of required information. Evidence: On June 14, 2016, the record for child #2 did not have documentation of emergency contact addresses. Plan of Correction: Program Director will do a training with registration staff and lead staff to make sure all of the forms are fully completed.
Standard #: 22VAC40-185-510-J Description: Based on observation, facility did not ensure medication is kept locked. Evidence: On June 14, 2016, an albuterol inhaler was stored in an unlocked filing cabinet in the office. Plan of Correction: Program Director will work with lead staff to include on their checklist that the medication cabinet is locked at all times.
Standard #: 22VAC40-185-550-M Description: Based on review, facility did not ensure 4 of 4 injury reports have documentation of required information. Evidence: On June 14, 2016, injury reports completed in May 2016 did not document parent notification (date, time and method), a second signature and plan for preventing recurrence. Plan of Correction: Program Director will do a training with registration staff and lead staff to make sure all of the forms are fully completed. Program Director will inquire with the association whether the incident form can be revised to include date and time instead of when.
