Comments:
An unannounced monitoring inspection was conducted from 9:23 a.m. - 1:30 p.m. There were seventy-five children in care with sixteen teachers working directly with them. There were two floaters and office staff to assist as needed. Five children's records were reviewed. The record for two teachers were reviewed. The younger children were observed being fed, during diapering, being played with and talked to by their teachers, playing with toys and babbling. The older children were observed having breakfast and lunch, during hand washing, playing outside, during soccer instruction, engaged in art projects making father's day gifts, talking and laughing and working at various play/work stations their teachers had set up for them. Standard #: 22VAC40-185-130-B
Description: Based on record review the facility failed to get updated immunizations for one child.

Evidence:

1. On 06/15/2016 the licensing inspector reviewed five children's records. The record for child #1 did not have updated immunizations.

2. On 06/15/2016 staff #1 confirmed there was no updated immunizations in the record for child #1.

Plan of Correction: The parent of child #1 was telephoned immediately. The parent had the updated immunizations but had failed to bring the documentation in for the record. The doctor's office promptly got the needed documentation to us for the record. We had the large paper clip on this record in order to monitor updates for immunizations.

Standard #: 22VAC40-185-270-C
Description: Based on temperatures taken there was one room that the temperature was lower than 68 degrees Fahrenheit.

Evidence:

1. On 06/15/2016 the licensing inspector took two thermometers and checked the temperature in room #1. The temperature was registering 64.1 degrees Fahrenheit and 65.6 degrees Fahrenheit in room #1.

2. On 06/15/2016 staff #2 and staff #3 confirmed the temperature readings taken in room #1.

Plan of Correction: Vents were closed and windows were opened in order to regulate the temperature. We will also purchase a new thermometer for the room so staff can monitor the room temperature and respond appropriately.
