22VAC40-191 Background Checks (22VAC40-191)

Technical Assistance:
We discussed foods that pose choking hazards for children under the age of three.

An unannounced monitoring inspection was completed. 89 children were in care; the age range of the children were three months to five years. Children were observed during outside time, doing art, lunch, and resting. Ten staff files and ten children files were reviewed. There are six staff are currently MAT trained. The inspection started at 10 AM to 4:15 PM.

Description: Based on the review of ten children's records not all were complete. Evidence: Child A with a start of 5/21/2015 and Child B with a start date of 1/13/14 did not have the physical address for both emergency contacts.

Plan of Correction: Center will obtain missing address for the emergency contacts from the parents.

Description: Based on review not all annual inspections were current. Evidence: The annual health inspection is dated 6/11/15.

Plan of Correction: Regional Director stated he will call the health department and schedule an inspection.

Standard #: 22VAC40-185-270-A
Description: Based on observation not all the inside and outside were maintained in a safe condition. Evidence: It was observed that the playgrounds the grass surface has worn to the point that the green netting was exposed posing a tripping hazard. The mats in the class room 2 was cracked. posing a tripping hazard.

Plan of Correction: Regional Director already contacted the landscaper to repair the playground area and the children will not be permitted to play on the grass area. The cracked mat was removed from classroom 2.

Standard #: 22VAC40-185-320-B
Description: Based on observations not all bathrooms were equipped with toilet paper. Evidence: The bathrooms in room 1 and 6 did not have toilet paper present in them.

Plan of Correction: Toilet paper was put in the bathrooms during the inspections.

Standard #: 22VAC40-185-340-A
Description: Based on observation children were allowed to drink from bottles while laying down. Evidence: In the infant room a 15 month old child was observed drinking a bottle while laying on a boppy pillow.

Plan of Correction: Center will review feeding procedures with staff in the infant room. Center will have the staff use high chair and feeding tables when children are drinking from bottles.

Standard #: 22VAC40-185-500-B
Description: Based on observation not all the diapering areas had foot operated trash cans available. Evidence: In room 2 and room 1 the staff were using regular trash cans without lids.

Plan of Correction: The center added the foot operated trash cans to the classroom during the inspection.

Standard #: 22VAC40-185-510-F
Description: Based on observation not all nonprescription medication was in the original container. Evidence: Child C's medication was missing the original container with direction label.

Plan of Correction: Center will request original box from parent. Center will no longer accept nonprescription medication without the original container with direction label.

Standard #: 22VAC40-185-560-F-4
Description: Based on observation children were being offered food that was a potential choking hazard. Evidence: The center was serving whole hot dogs to children under the age of three.

Plan of Correction: The center cut the hot dogs into smaller pieces for all children during the inspection. The center will be removing hot dogs from their menu.
