Comments:
The LI conducted an unannounced monitoring inspection in the Center on June 14, 2016. As reported, 36 children are currently enrolled in the center. The LI observed 30 children grouped in 3 classrooms, infant room (7 children/2 staff), toddler room (7 children/2 staff), pre-K classroom (16 children/3 staff). The children's ages ranged from infancy (3 months) to five years. Three additional support staff were also present in the Center. A sample of 4 children's records was reviewed. Two partial staff records were reviewed. Medications and written parental authorizations were reviewed for 3 children. The LI reviewed current medication certificates (MAT) for 4 staff members. Documentation of the Center's annual fire and health inspections was current. Documentation of the practiced emergency drills was complete. Staff and children were observed during varied activities/services which included: outdoor play, diapering procedures, hand washing, the lunch meal, nap/rest time, and free play. Staff were observed interacting with the children and were attentive to their needs. Findings of the inspection were reviewed with the Education Coordinator at the exit. Standard #: 22VAC40-185-60-A
Description: Based on a sample review of children's records, the Center failed to ensure complete information was maintained in each record. Evidence: Four children's records were reviewed, the following information was incomplete: the name, home address and phone number of each parent who has custody, when applicable, work phone number and place of employment of each parent who has custody; allergies/any chronic physical problems (child 3)

Plan of Correction: Children's records will be reviewed by the Center's Family Development Specialist to ensure the required information is maintained in each record.

Standard #: 22VAC40-185-90--A
Description: Based on reviewed documentation, the written agreement between the parent and the center did not contain a statement that the parent would immediately inform the center of life threatening diseases developed by his child or any household member. Evidence: The LI reviewed a sample of 4 children's records, two of four records did not contain the revised parental agreement (children 1 & 4).

Plan of Correction: The FDS will review the revised parent agreement with each party and will maintain a signed copy in each child's record.

Standard #: 22VAC40-185-430-A
Description: Based on direct observation, the Center staff failed to ensure the protective belt was securely fastened while an infant was placed in an infant seat. Evidence: The LI observed a staff member in the enclosed area of the infant room while supervising 3 infants. One child (3 months old) was seated in an infant seat placed on the floor. The protective belt was not fastened and was observed folded under the child.

Plan of Correction: Staff will be reminded to always use protective belts whenever children are placed in infant equipment. Classrooms will be monitored on-going.

Standard #: 22VAC40-185-430-O
Description: Based on the LI's observations, the Center failed to assure a portable water cooler used was designed with a cover and that the water may be withdrawn from the container only by a water tap or faucet. Evidence: The LI observed a water pitcher on the playground which was used to pour water into cups for the children during outdoor play.

Plan of Correction: A covered water cooler will be provided for transporting and serving water to the children during outdoor play.
