An announced, on-site mandated move inspection was initiated and completed on 10/16/2025. The on-site inspection began at 9:30 am and ended at 10:30 am. The inspector reviewed compliance in the areas listed above. There were 2 children present and 1 caregivers; the point total was 5. The inspector reviewed 6 children?s records and 3 caregiver/household member(s) records on-site. This inspection included document review, a tour of the home, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program

Standard #: 8VAC20-800-60-B Description: REPEAT VIOLATION
Children's records are required to contain two emergency contacts.
The record for Child #2 and Child #6 were missing 1 out of 2 emergency contact information.Plan of Correction: I?ll have parents update their emergency contacts by 10/17/2025.
Standard #: 8VAC20-800-650-H Description: Parents shall be informed of any changes in the home's emergency preparedness and response plan.
The records for Children #1 through #6 contain parent-signed emergency preparedness and response plans; however, the documented relocation address reflects an outdated location.Plan of Correction: I?ll notify parents of the changes in the emergency prep and response plan.
