Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Comments:

An unannounced, on-site monitoring inspection was initiated and completed on October 23, 2025, as a part of the licensure period. The on-site inspection began at 8:30 a.m. and ended at 9:30 a.m. The inspector reviewed compliance in the areas listed above. There were 8 children present and 3 caregivers; the point total was 21. The inspector reviewed 4 children?s records and 3 caregiver/household member(s) records on-site. This inspection included document review, tour of the home, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program. Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on October 30, 2025. A POC submitted after this date will not appear on the public website.

Standard #:	8VAC20-800-780-B
Description:	Annually, the provider shall review with the parent the emergency contact information is correct, and obtain the parent's signed acknowledgment of the review. The record for Child #1, enrolled for 2 years, was missing an annual review and was overdue by over 3 weeks.
Plan of Correction:	The parents will review the information today and sign acknowledging their review.
Standard #:	8VAC20-800-250-C
Description:	Items tied across the top or corner of a crib shall be removed when the child begins to push up on hands and knees or is five months of age, whichever occurs first. Child #2 (1 year old) and Child #4 (10 months old) were asleep in cribs and blankets were tied to each end.
Plan of Correction:	The tied blankets were immediately removed from the cribs during the inspection.
