Comments:
An unannounced monitoring inspection was conducted today, June 9, 2016 to determine the center's compliance with licensing standards. Upon arrival, eighteen children ages 2-5 years were observed playing on the playground being supervised by three staff. Ten school age children being supervised by one staff were located in the school age building participating in arts & crafts. All areas used by children were inspected to include the main hallways, children restrooms, classrooms, school age building, and playground. See attached violation notice for hazards found on this day. The center had the required postings displayed on the wall upon entering the building. The center's first aid kit and emergency supplies were inspected with the required supplies found. Four children and two staff records were inspected with the required information present. The following was also inspected on this day: annual fire & health inspections, fire/shelter-in-place drills, emergency preparedness plan, daily attendance log, parent agreements, and other pertinent information. Description: Based on the inspection of center, two areas were found in an unsafe condition. Evidence: During the inspection of the center, the children's porch located on the front side of the building has carpet that appears to be unraveling causing a tripping hazard to children sitting or playing on the porch. Also on the children's playground, the tubular crawler has cracked and chipped paint.

Plan of Correction: Per director: We have ordered carpet and awaiting installation which should be by 6/30/16. The children's playground tube will be painted by 6/23/16.
