Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Comments:

An unannounced, on-site monitoring inspection was initiated on 10/15/25 and completed on 10/15/25. The on-site inspection began at 10:50 a.m. and ended at 11:40 a.m. The inspector reviewed compliance in the areas listed above. There were seven children present and one caregiver; the point total was 14. The inspector reviewed five children?s records and one caregiver record on-site. This inspection included document review, tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program. Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 11/3/25. A POC submitted after this date will not appear on the public website.

Description:	Providers and substitutes shall have current certification in cardiopulmonary resuscitation (CPR). Evidence: The CPR certification for Caregiver #1 expired on 6/2025. The certification was overdue by four months
Plan of Correction:	Same as above. I will also ensure not to let certificate/certifications lapse.

Description:	Providers and substitutes shall have current first aid certification. Evidence: The First Aid certification for Caregiver #1 expired on 6/2025. The certification was overdue by four months.
Plan of Correction:	CPR/First Aid training rescheduled from 10/8/25 to 10/25/25 due to death in trainer family.
