Comments:
An unannounced complaint inspection was initiated on June 7, 2016 and completed on June 24, 2016. The complainant reported concerns regarding special care provisions and emergencies. An inspection of the facility was conducted, documentation was reviewed and obtained and interviews were held with staff and other collateral witnesses. Based on gathered information, there is a preponderance of evidence to support the allegation, therefore this complaint has been determined to be VALID. There were other violations found that were not reported. Areas of noncompliance are identified on the Violation Notice.

Complaint related: No
Description: Based on record review, in four of four children's records reviewed, the licensee did not ensure that each child shall have a physical examination by or under the direction of a physician before the child's attendance or within one month after. Evidence: During the inspection conducted on June 7, 2016, there was no documentation of physical examinations in the records for Child #1-Child #4 (start date as indicated in the record for these children was 1/20/15).

Plan of Correction: In the future we will ensure that applications are filled out upon enrollment and all documents are received before attendance.

Description: Based on record review, in three of four children's reviewed, the licensee did not ensure that each enrolled child had a record that contained all required information. Evidence: During the inspection conducted on June 7, 2016, the records for Child #2-Child #4, did not include viewing proof of the children's identity and one parent's name, address, and employment information.

Plan of Correction: In the future we will ensure that applications are filled out upon enrollment and all documents are received before attendance.

Complaint related: Yes
Description: Based on inspection of the facility and interviews with staff, the licensee did not ensure that when children at the center have been exposed to a communicable disease listed in the Department of Health's current communicable disease chart, the parents shall be notified within 24 hours or the next business day of the center's having been informed unless forbidden by law, except for life threatening diseases, which must be reported to parents immediately. Evidence: During the inspection conducted on June 7, 2016, it was verified through multiple staff interviews that parents were not notified within 24 hours or the next business day when there was at least one confirmed case of a communicable disease that affected both buildings at the facility. According to documentation obtained from the center, the communicable disease was confirmed by a physician on 5/16/16.

Plan of Correction: We will ensure that all illnesses are posted immediately upon notification.
