Reviewed with the director that staff references taken over the phone must be clearly documented that they were taken by phone.
An unannounced inspection for license renewal was conducted today. Ratios for all seven groups were found in compliance. The children were observed playing outside and eating lunch. One group was practicing for their end of the year performance. Staff with MAT certification, first aid and CPR training were on site. Currently there is no medication on site for the children. As we discussed, please rake the mulch to the fall zones under the equipment on the playground where it has worn down. If you have any questions, I can be reached at 703-479-4709.
Standard #: 22VAC40-185-140-A Description: Based on record review, no documentation of a physical exam was found for one child who has been in attendance for more than one month. Plan of Correction: Documentation of a physical exam for this child will be obtained from the parent.
Standard #: 22VAC40-185-270-A Description: Based on observation, areas and equipment of the center were not maintained in a safe and operable condition. Findings: At the time of the inspection, the inspector found the following: 1. The material under the mulch on the playground is coming up through the mulch and bunching up, causing a possible tripping hazard for the children. 2. The toilet paper holder in the bathroom in the Montessori II room is broken off the wall. Plan of Correction: 1. The material will be buried and covered with mulch so it is no longer coming up and causing a hazard. 2. The toilet paper holder will be repaired.
