Comments:
An unannounced monitoring inspection was conducted on July 13, 2016. At the onset of inspection, four staff were present with 27 children. Children were observed doing a cooking activity, playing on the playground and doing tabletop activities in the cafeteria. Three children's records were reviewed on this date. Licensing Inspector reviewed supervision, activities, equipment, hand washing, nutrition, injury report documentation, medication procedures, children's records, emergency evacuation and shelter-in-place drill documentation, fire and health inspection reports, liability insurance coverage, emergency supplies and required postings. Violations were reviewed with and verified by the Program Director.

Standard #: 22VAC40-185-510-E
Description: Based on observation and inspection of the center, the staff members did not ensure that medication shall be labeled with the child's name, the name of the medication, the dosage amount, and the time or times to be given. Evidence: During the inspection conducted on July 13, 2016, an emergency medication was not labeled with the child's name.

Plan of Correction: I will have the parent provide the prescription label.

Standard #: 22VAC40-185-560-N
Description: Based on observation and inspection of the facility, the staff members did not ensure that no child shall be allowed to drink or eat while walking around. Evidence: During the inspection conducted on July 13, 2016, the licensing inspector observed six children drinking water while walking around and playing on the outdoor playground equipment.

Plan of Correction: I will notify staff and talk to the children.
