Additional staff may be required in rooms with special needs children. A physician should determine if a smaller class size or individualized attention is required.
An unannounced renewal inspection was conducted with assistant director and center staff. There were 91 children present during today's inspection. The children were engaged in a variety of activities including a clay project, various manipulatives including Legos, blocks, and cars, and snack time. Snack provided by the center today: applesauce and graham crackers. Two staff files and one child's file were reviewed and are in compliance. Two medications were reviewed - please be sure all medications, including prescription diaper creams, are administered by a certified Medication Administration Trained staff. Standard #: 22VAC40-185-240-D-1 Description: Based on observation and staff interview, the facility failed to have a staff member who has satisfactorily completed Medication Administration Training to administer medications. Evidence: a staff person who has not been certified in Medication Administration Training administered a prescribed medicated cream, Nystatin, to an infant during diaper change. Plan of Correction: A staff person who is certified in Medication Administration Training will administer all medications.
Standard #: 22VAC40-185-500-A Description: Based on observation, the facility failed to wash each child's hands after diapering. Evidence: after a diaper change in the infant room, the infant was placed on the floor without staff assisting with handwashing. Plan of Correction: All children's hands will be washed with soap and water after toileting/diaper change.
Standard #: 22VAC40-185-510-L Description: Based on observation, the facility failed to maintain a record of each medication given to children while in care. Evidence: there was no medication log detailing the amount of medication administered, date and time administered, staff member administering, any adverse reactions or medication error for a medicated cream, Nystatin. Plan of Correction: The facility will begin logging the medicated cream with the required information on the medication log.
