Though the main bathroom is equipped with six toilets, separating the children by gender to use the facilities may result in an accident. If the center prefers boys and girls use the main bathroom separately, it may be necessary to put up a barrier to separate toilets in a way that allows sight and sound supervision, but gives the children a sense of privacy. Several boys had to wait for 10 minutes to use the restroom after rest because there were girls in the restroom.
An unannounced monitoring inspection was conducted with center director and center owner. There were 58 children present, ranging in age from 3 months to 12 years. The children were engaged in a variety of activities including outdoor play, reading, free play, and afternoon snack. Snack served today: juice and graham crackers. The last fire inspection was conducted 3/3/2016. The last health inspection was conducted 12/21/2015. Standard #: 22VAC40-185-270-A Description: Based on observation, the facility failed to keep all areas outdoors maintained in a clean, safe, and operable condition. Evidence: there was trash, deflated balls/pools, and a wrench on the playground in the following areas: toddler playground (around air conditioning unit, school age playground (around air conditioning unit, and preschool playground (near the hose). Plan of Correction: Staff will pick up trash and broken materials today.
Standard #: 22VAC40-185-330-B Description: Based on observation, the facility failed to maintain resilient surfacing in all fall zones on the playground. Evidence: there was not sufficient mulch under and in the full trajectory area of the swings on the playground. Plan of Correction: Center owner has contacted a landscape person to come disperse mulch in the fall zones of the swings and other playground equipment as needed.
Standard #: 22VAC40-191-40-D-1-B Description: Based on review of staff records, the facility failed to have a completed search of the central registry Child Protective Services check before 30 days of employment at the facility ends. Evidence: Of three files reviewed, Staff C (date of hire 3/7/2016) did not have documentation that the search of the central registry was completed. Plan of Correction: Center director will look for the completed search of the central registry - it has come in the mail but not been filed.
