22VAC40-191 Background Checks (22VAC40-191)

This unannounced monitoring inspection was conducted from 9:45 A.M. until 11:45 P.M. At the time of the inspection 62 children were present with eight staff. Children and staff were observed during free indoor and outdoor play, structured indoor play, an art activity, a cooking lesson, bathroom break, and hand washing. Staff were observed during transitions and behavioral management. The sample size consisted of four children's records and one child's medication. The violations from the previous inspection were reviewed for current compliance. No repeat violations were found. One violation was found during this inspection and can be found on the violation notice. Description: Based on record review, the administration failed to document they reviewed the proof of age and identity of one out of four children. Evidence: The record for child 4 did not have documentation that the proof of age and identity was reviewed.

Plan of Correction: The parent of child 4 will be advised to provide documentation of the child proof of age and identity.

Standard #: 22VAC40-185-60-A-8
Description: Based on record review and interview the administration failed to ensure the parent of a child in care documented special accommodations a child needed while at the center for one out of four records reviewed. Evidence: 1. The record for child 5 did not contain accommodations the child needed for medication the child self administers while at the center. 2. Staff 1 verified the parent did not document the specific accommodations were not recorded in the child's record.

Plan of Correction: A medication management plan was obtained 7/25/16.
