Comments:
An unannounced monitoring inspection was conducted on July 7, 2016, from approximately 12:20PM to 2:00 PM. There was 6 staff were present with 14 children. Children were observed napping and freely playing. There were other children and staff on a field trip. Three children records were reviewed on this date. Licensing Inspector reviewed supervision, activities, equipment, nutrition, special feeding, injury report documentation, children's records, emergency evacuation and shelter-in-place drill documentation, health inspection report, emergency supplies and required positing. No medication is given at the center. The exit interview conducted with the staff member in charge . The licensee's response for the "plan of correction" was not received as of July 27, 2016 and will not appear on this Violation Notice.

Description: Based on observation, a staff record was not accessible. Evidence: During the inspection conducted on July 6, 2016, Staff #1 who was in charge was not able to provide the licensing inspector with staff records. The records was kept locked no staff other than owner had access to the record.

Description: Based on record review and interview, the licensee did not ensure there was at least one staff member on duty who has obtained within the last three years instruction in performing the daily health observation of children. Evidence: During the inspection conducted on July 6, 2016, the documentation was not available to verify there was a staff member on site who obtained daily health observation in the last three years. The staff records were kept locked and staff present were not able to access the staff records.

Plan of Correction: For privacy if staff record has always been locked up will come up with another plan to keep records

Description: Based on record review, the licensee did not ensure the fire inspection was done annually. Evidence: During the inspection conducted on July 6, 2016, the center was unable to provide documentation of the last fire inspection.

Description: Based on observation, the licensee did not ensure the outside , was maintained in a clean, safe and operable condition. Evidence: During the inspection conducted on July 6, 2016, the following was observed on the outside area on the playground: 1- There was a Fisher Price climbing rocket equipment piece for 18 months - 6 years is cracked and have sharp edges. 2-There was pink cozy coupe had a plastic white piece that is broken with sharp edges. 3-There was a wooden picnic table the wood is splintering on the bench.

Description: Based on observation, the licensee did not ensure electrical outlets had protective covers. Evidence: During the inspection on July 6, 2016, the licensing inspectors observed a power strip by the refrigerator missing 4 electrical outlet did not have protective covers .

Description: Based on observation, the licensee did not ensure the restroom was equipped with toilet paper. Evidence: During the inspection on July 6, 2016, the Licensing Inspector observed in the bathroom where the school age were sleeping the restroom did not have toilet paper.

Description: Based on observation and inspection of the playground, the licensee did not to ensure that the required amount of resilient surfacing was under playground equipment with moving parts or climbing apparatus to create a sufficient fall zone. Evidence: During the inspection conducted on July 6, 2016, there was an inadequate amount of resilient surfacing in the fall zone under and surrounding the arch climber . The depth of the mulch was unmeasurable. The mulch has decomposed into soil.

Description: Based on observation, licensee did not ensure there is at least 12" of space between occupied cots during nap time. Evidence: During the inspection on July 6, 2016, Licensing Inspector observed in the classroom 8 school age children napping. Seven of the eight cots were less than 12 inches a part.

Description: Based on staff interview, the licensee did not ensure there was member trained in first aid and CPR on premises during the center's hours of operation. Evidence: During the inspection conducted on July 6, 2016, the documentation was not available to verify there was at least one staff member on site who obtained first aid, cardiopulmonary resuscitation, and rescue breathing . The staff records were kept locked and staff present were not able to access the staff records.
