- Technical Assistance:
Today we discussed including children with special needs and the use of "google" to determine the age rating for video games. Additionally, today risk rating and the potential affect of risk rating on future licenses was discussed.
An unannounced inspection was conducted today from 1:00 - 1:30 and from 2:30 - 4pm. There were 11 children in care with 2 staff at the onset of the inspection. Children and staff records were reviewed. Required postings were reviewed. Children were observed in teacher and self-directed activities including nap, snack, restroom routines, transitions and pick-up. Triple digit temperatures prevented staff from taking children outside this afternoon. Violations regarding physical plant, children's records, sleep/rest and programs were cited and appear on the violation notice.
Standard #: 22VAC40-185-130-A Description: Based on record review and interview the center failed to ensure that documentation of required immunizations are obtained before the child can attend the center. Evidence: 1 - The record for child 1 does not contain an immunization record. a. The child has a documented date of enrollment as 2/10/16. 2 - Staff 1 confirmed that the record for child 1 does not contain an immunization record. Plan of Correction: Center management will ensure that immunization records are obtained prior to enrollment as required. The information will be obtained from the parent.
Standard #: 22VAC40-185-140-A Description: Based on record review and interview the center failed to ensure that a physical examination is obtained within 30 day's of the first day of attendance. Evidence: 1- The record for child 1 does not contain a physical examination record. a. The child has a documented date of enrollment as 2/10/16. 2 - Staff 1 confirmed that the record for child 1 does not contain a physical examination record. Plan of Correction: Center management will ensure that a physical is obtained with in 30 days of enrollment as required. The information will be obtained from the parent.
Standard #: 22VAC40-185-270-A Description: Based on observation and interview the center failed to ensure that areas are maintained in safe condition. Evidence: 1 - Staff 1 sprayed the snack table with bleach water solution while three children were sitting no more than one foot away from the table. 2 - Staff 1 confirmed that the children were sitting at the table when as she prepared the table for snack with bleach water solution. Plan of Correction: Center management will remind and retrain all staff in the need to keep children away from all areas where chemicals are being used.
Standard #: 22VAC40-185-360-A Description: Based on observation and interview the center failed to ensure that daily activities are age and stage appropriate. Evidence: 1 - Three children were playing a video game with an age rating of "Teen". The game required classic super hero characters to engage in hand-to-hand combat and use weapons to destroy each other. a. Child 3 stated that he was 5 years old b. Child 4 stated that he was 11 years old c. Child 5 stated that he was ten years old. 2. Staff 2 confirmed that the children were not teenagers. Additionally, Staff 2 stated that he did not have the "jacket" for the game and did not know the age rating for the game. Plan of Correction: Center management will review the age rating on all video games prior to allowing children to use the games. Games that center management are unable to determine the age rating for will not be used by children in care at the center.
Standard #: 22VAC40-185-450-A Description: Based on observation and interview the failed to ensure that cots used by children have a bottom cover. Evidence: 1 - Child 2 was napping on a cot without a bottom cover. 2 - Staff 1 confirmed that child 2 was napping without a bottom cover. The staff stated that the parent had not sent a clean sheet to the center that morning. Plan of Correction: Center management will ensure that each child has bottom cover. If the cover is not provided by the parent as required by the center, the center will give the child a cover to use while in care.
