Technical Assistance:

The licensing inspector conducted an unannounced monitoring inspection on July 26, 2016 from approximately 10:00am to 12:45pm. The following ratios were observed with a total of 54 children present during the inspection:
Infants - 8 children with 2 staff
Toddlers - 6 children with 2 staff
Twos (Discovery Preschool) - 10 children with 2 staff
Threes (Preschool) ? 9 children with 1 staff
Fours & Fives (Pre-K) - 10 children with 1 staff
School Age children - 11 children with 1 staff

During the inspection, the Infants were observed having their diapers changed, looking at picture cards while the staff used descriptive words to describe the items, playing on the floor with toys and sleeping in their cribs. The Toddlers were observed playing outside with bubbles and toy trucks, looking at books, working puzzles, and having their diapers changed. The Twos classroom was observed having water play with sprinklers on the playground. The Preschool classroom was observed playing with musical instruments, looking at plastic bugs with magnifying glasses, building with magnetic blocks, playing with magnetic letters and building with legos. The Pre-K classroom was observed washing their hands, playing with magnets, learning about authors and illustrators and participating in Stretch-N-Grow program. The school age children were observed playing board games, building with blocks, playing with Barbies and creating birds with stencils, construction paper and feathers. Staff was observed interacting with the children and providing assistance when needed. Lunch was served during the inspection and consisted of pizza, green beans, pineapples and milk. Later children were observed transitioning into naptime. All areas of the center including classrooms, hallways, bathrooms, kitchen, and playgrounds were inspected. The center was equipped with toys and supplies and items were available to the children.

The following information was reviewed: emergency supplies, injury reports, fire and shelter-in-place drills, daily attendance, daily sheets, parent agreements, and staff training information. Medication is administrated. Medication and authorization forms were reviewed. Transportation is provided and vehicles and transportation documents were inspected.

Standard #: 22VAC40-185-60-A
Description: Based on review of five children's records, two records did not contain the required information. Evidence: During the inspection, the licensing inspector observed the record for Child #4 (first date of attendance 03/11/15) did not contain a physical address for one of the two emergency contacts listed. The record for Child #5 (first date of attendance 04/20/11) did not contain documentation of annual update of the child's information in the record. The last review / update of the child's record was documented as occurring in 2013.

Plan of Correction: Per management - We will obtain the required information and add it the children's records.

Standard #: 22VAC40-185-270-A
Description: Based on inspection of the building, the facility did not ensure all areas and equipment were maintained in a safe and operable condition. Evidence: During the inspection, the licensing inspector observed two metal brackets protruding over the top of the boys toilet located in the school age boys bathroom. The metal brackets were rusted and had sharp edges creating a hazard.

Plan of Correction: Per management - I will remove the brackets from the bathroom.
