Comments:
An unannounced monitoring inspection was conducted on July 22, 2016. Upon arrival to the facility, three staff were present with 15 children. An additional staff person arrived throughout the course of the inspection. Children were observed playing a variety of tabletop activities to include board games, card games, coloring books and Dominoes. They were also observed making strawberries dipped in white chocolate and talking freely amongst themselves. Three children's records were reviewed on this date. Licensing Inspector reviewed supervision, indoor and outdoor areas, activities, equipment, nutrition, injury report documentation, children's records, emergency evacuation and shelter-in-place drill documentation, fire and health inspection reports, liability insurance coverage, emergency supplies and required postings. Medications are not being administered at this time.

Violations:
Standard #: 22VAC40-185-60-A
Description: Based on record review, in two of three children's records reviewed, the licensee did not ensure that each child's record was documented with all required information. Evidence: During the inspection conducted on July 22, 2016, the record for Child #1 did not include documentation of the work phone number for both parents and the record for Child #2 did not include documentation of the work phone number for one parent.

Plan of Correction: I will ask the parents for the missing information and have the files updated.

Standard #: 22VAC40-185-70-A
Description: Based on review and inspection of the facility, the licensee did not ensure that all information required for each staff record was kept at the center. Evidence: During the inspection conducted on July 22, 2016, the name, address, verification of age requirement, job title, and date of employment or volunteering; and name, address and telephone number of a person to be notified in an emergency for Staff #2 was not kept at the center.

Plan of Correction: We will get in contact with the other assigned site the employee works at and get a copy of all missing information.

Standard #: 22VAC40-185-240-D-5
Description: Based on observation and inspection of the facility, the licensee did not ensure that there shall always be at least one staff member on duty who has obtained within the last three years instruction in performing the daily health observation (DHO) of children. Evidence: During the inspection conducted on July 22, 2016, upon arrival to the facility, there were three staff members present. There was no documentation that the staff members had obtained DHO training.

Plan of Correction: I will speak with an administrative staff about taking the DHO course to receive the qualifications. A staff member arrived during the inspection with documentation of current training.

Standard #: 22VAC40-185-530-A
Description: Based on review and inspection of the facility, the licensee did not ensure that there shall be at least one staff member trained in first aid, cardiopulmonary resuscitation (CPR), and rescue breathing as appropriate to the age of the children in care who is on the premises during the center's hours of operation and also one person on field trips and wherever children are in care. Evidence: During the inspection conducted on July 22, 2016, upon arrival to the facility, there were three staff members present. There was no documentation that these staff members had received first aid and CPR training.

Plan of Correction: I will speak with an administrative staff about taking the course to receive the qualifications. A staff member arrived during the inspection with documentation of current training.

Standard #: 22VAC40-185-550-D
Description: Based on review and inspection of the facility, the licensee did not ensure that the center implemented a monthly practice evacuation drill. Evidence: During the inspection conducted on July 22, 2016, there was no documentation of evacuation drills practiced in April and May of 2016.

Plan of Correction: I will contact an administrative staff regarding the fire drills and make sure drills are done and documented monthly.
