Comments:
An unannounced monitoring visit was done today July 28, 2016 beginning at 10:10 AM and ending at 1:30 PM. There were seven children in care today during the summer program. The children were playing outside when the inspector arrived. They were also observed during their classroom time when the teacher read a story to them and then they worked on a craft. Lunch and nap were also observed. A fire drill was practiced today during the inspection. The inspector also reviewed a sample of staff and children's files. The outside play area was inspected today as well. The inspector discussed the violation and corrective action with the owner during the exit interview. Consultation was given in regards to documentation for insect repellent and medication administration. Remind staff to sit with the children during their lunch time and to keep a check on the labeling of sunscreen/insect repellents.

Description: Based on observation and discussion the center did not maintain the resilient surfacing at the proper depth. Evidence: 1. During the inspection the inspector measured the wood chips in the fall zones around climbing pieces of equipment and it was measured to be between 1-4 inches and not at a required six inches of depth.

Plan of Correction: The children will not play on the equipment until the wood chips are at the required six inches of depth. This will be corrected by adding additional wood chips.
