Today an unannounced renewal inspection was conducted with the Director. This visit took place in the morning between 9:00am-12:30 there were 9 classrooms observed and the summer camp for a total of 180 children with 36 staff within the supervision guidelines. The children were observed having morning outdoor play, arriving to school, making super hero costumes, story-time, morning snack, arts & craft and in transition for lunch time. Positive interactions were observed during this inspection. A complete inspection of the physical plant, children and staff records, fire drill log, liability insurance, playground procedures, medication and emergency procedures were observed during this inspection. Standard #: 22VAC40-185-60-A Description: Based on a review of the children records, the two of the records did not include the parents complete work address and documentation of two or more designated emergency contact person's address. Evidence: Child #3's record did not include mom's complete work address and 1 emergency contact person's complete address Child #5's record, did not include the father's work address. Plan of Correction: Center will contact the parent today to request the information and will verify when the forms are received that all of the licensing information is included.
Standard #: 22VAC40-185-270-A Description: Based on observation of the physical plant, the following areas were not being maintained in a clean, safe and operable condition. Evidence: In 3 out of 9 of the classrooms observed, visible cracks were found along the border of the wall base boards with the the borders coming apart. In the 2 year old classroom, the carpet was ripped/torn and being tapped to keep it together however the tape is ripped and coming apart and therefore a tripping hazard to the children. Plan of Correction: Center Director will submit these items to the maintenance team for them to fix them.
Standard #: 22VAC40-185-550-M Description: Based on a review of the injury reports, a random sampling of the injury reports did not include the following information. Evidence: 10 injury reports did not include the Date and time parents were notified. Plan of Correction: Center Director will do a center wide review on accident/incident report and what needs to filled out on the forms and why it is important.
