During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Technical Assistance:
Technical assistance provided for the renewal application. Provider is submitting today for renewal. Discussed sworn disclosure statements, staff records, and the required information to keep. Emailed provider model forms and information to use in records. Technical assistance provided for indoor soft climbing equipment which provider will replace if becomes ripped or torn.

Comments:
An unannounced, on-site renewal inspection was initiated and completed on December 9, 2025, as a part of the licensure period. The on-site inspection began at 9:15 a.m. and ended at 11:15 a.m. The inspector reviewed compliance in the areas listed above. There were 10 children present and 3 caregivers; the point total was 34. The inspector reviewed 5 children?s records and 4 caregiver/household members records on-site. This inspection included document review, tour of the home, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on December 16, 2025. A POC submitted after this date will not appear on the public website.

Standard #: 22.1-289.035-B-1
Description: The family day home is required to obtain a completed sworn statement prior to a caregiver?s first day of employment.

The sworn statement for Caregiver #3, who is currently working, was dated six weeks after their first day of employment.

Standard #: 8VAC20-800-920
Description: When meals or snacks are provided by the family day home, the menu for the current one-week period shall be dated and given to parents or posted.

The menu for the current week was not posted and had not been given out to parents.

Description: Bottles shall be refrigerated and labeled with the child's full name and the date, if more than one infant is in care.

Two bottles for Child #1 (9 months old) were missing the date and one bottle for Child #2 (22 months old) was missing the child?s name and date.

Description: Providers must maintain a record for each caregiver with the required information.

The record for Caregiver #3 (employed over 2 months) and the record for Caregiver #4 (employed over 1 month) was missing their verification of age, date of employment, and the name, address, and telephone number of an emergency contact.

Plan of Correction: updated assistant/substitute provider record forms for Caregiver #3 and Caregiver #4 with all up to date information attaching proof of age verification, date of employment, and emergency contact information.

Description: Information contained in a child's record shall be privileged and confidential.

The provider had the supplemental page from the previous inspection posted on the parent board that contained confidential information.

Description: Providers shall ensure that small electrical appliances are unplugged unless being used.

The blender in the upstairs kitchen was plugged in and not in use.
