Comments:
Conducted an investigation into a self-reported incident of a lack of supervision. Records reviewed, staff and involved parties interviewed. As a result of the investigation into the incident, violations were cited. Questions about this report may be directed to: pamela.sneed@dss.virginia.gov

Violations:
Standard #: 22VAC40-185-40-E
Description: Based on interviews conducted, it was determined that staff did not follow the center's policies and procedures for accounting for the children in their care on 7/26/16.

Evidence:

1. The center's policies/procedures require staff to account for children by conducting a name-to-face roll call check during transition periods, to include, when a staff person enters the classroom, and when children move from one location to another.

2. Staff #1 did not complete the name-to-face roll call check when she entered the classroom, or prior to exiting the building for the emergency evacuation drill.

Standard #: 22VAC40-185-340-A
Description: Based on interviews conducted and records reviewed, it was determined that staff did not ensure the care and protection of a 4 year-old child that was left alone and unsupervised in the center during an emergency evacuation drill on 7/26/16. Evidence: 1. Staff #1 stated that she arrived to the classroom at approximately 2:05pm, to cover the lead teacher's break. Some children were not napping, she got distracted playing a game with them, and forgot to sign-in and do the name-to-face attendance, as required by center policies, and as a result did not know which children were sleeping. She stated, "I wasn't aware of the other kids in the classroom." She did not notice Child #1, who is "100% deaf," sleeping on the cot because the room was dark and her entire body was covered with a blanket, and stated, "I didn't see her." 2. When the emergency alarms sounded, Staff #1 proceeded to exit the building with all children in the classroom except Child #1. She explained that she assumed there were 10 children present because that was the normal number, and didn't know that on this day there were 11 children in her care. She said that she did a visual count of 10 children and believed she had all of the children in her group. 3. Staff #2 stated that she did not notice Child #1 sleeping on the cot when she did a "sweep" of the center, as the last person to exit the center. She said that she was "focused on checking bathrooms and closets." 4. The staff were not aware the child was missing until Staff #1 completed the name-to-face attendance at the evacuation location, saw Child #1's name on the list, and realized she had 10 of the 11 children on her attendance list. When asked by Staff #3 if she had all 11 children, she confirmed that she did "to keep them from panicking." She did not alert anyone to the child's absence. 5. Upon completion of the drill and return to the center, Staff #1 found Child #1 sleeping on the cot. Staff #3 entered the classroom and observed what she thought appeared to be Child #1 waking up, proceeded to ask Staff #1 if the child had been left inside during the drill, and Staff #1 stated "Yes." 6. According to the building engineer report, the emergency evacuation drill was initiated at 2:21pm and concluded at 2:31pm. 7. Staff reported the total time the drill took for them to wake children, exit the building, walk to the upper level of the parking garage, exit to the designated exterior evacuation location, wait for the all clear, and return with the children to the center was between 15-20 minutes.

Plan of Correction: Director will review our policy and procedures on fire drills + evacuation during the monthly staff meeting schedule on 8/9/16. Training will include procedures on transition tracking (name to face roll call check), through the door training including a video on supervision and a review of the "fire drill + evaluation" policy + procedure. 8/9/16 Director will review the "sweep" procedures with her administrative team or those acting as manager in her absence. 8/11/16 Regional Manager reviewed proper fire drill + evacuation procedures with Director and reviewed Director job description. 8/8/16
