An unannounced monitoring inspection was conducted on August 10, 2016 from approximately 11:15 am to 1:25 pm. Nine staff were present with fifty-three children. Additional staff were on site for other responsibilities. Children were observed during free play, story time, lunch and nap time. Licensing Inspector reviewed standards related to supervision, activities, equipment, supplies, nutrition, special feedings, emergency evacuation and shelter-in-place drill documentation, fire and health inspection reports, injury reports, liability insurance coverage documentation, children & staff records, emergency supplies and required postings. Violations were reviewed with and verified by the program director during the exit interview conducted on this date.
Standard #: 22VAC40-185-280-B Description: Based on observation, facility did not ensure hazardous substances are kept locked. Evidence: On August 10, 2016, a spray bottle with a clear liquid labeled "Sanitizer" was stored on the top shelf of a red cubbies. Plan of Correction: The bottle will be locked away today.
Standard #: 22VAC40-185-440-B Description: Based on observation, facility did not ensure rest mats are identified for use by a specific child. Evidence: On August 10, 2016, 2 children in the Windows classroom were confirmed to be sleeping on a mat labeled for a different child. Plan of Correction: Staff will make sure adequate mats are ordered so that the children can sleep on the mat they are supposed to sleep on.
Standard #: 22VAC40-185-570-E Description: Based on observation, facility did not ensure children's bottles are labeled with the child's name and date. Evidence: On August 10, 2016, 2 of 5 prepared bottles in the infant room refrigerator were not labeled with the date. Plan of Correction: Staff will date them.
