- Technical Assistance:
1. Discussed importance of reviewing staff Sworn Disclosure Statements to ensure satisfactory completion. 2. Exterior garden hoses should removed from path of travel when not in use.
Conducted an unannounced monitoring inspection 11:05am-12:25pm. Observed 16 children + 4 direct-care staff. Ratios were well in compliance and supervision was constant. Children were observed doing writing and spelling activities, and napping during the inspection. The center was found to be very clean, safeguarded appropriately, and well supplied with toys and equipment for the children. Lunch served today included: chicken, potatoes, corn, and milk. No medications reported to be on-site. Required documents were posted. Record keeping violations found. Questions about this inspection may be directed to: firstname.lastname@example.org
Standard #: 22VAC40-191-60-B Description: Based on records reviewed, it was determined that 2 of 4 staff records did not include a satisfactory Sworn Disclosure Statement (SDS). The SDS documents on-file for both staff persons were falsified, as related to their criminal history. Evidence: 1. Staff #3 - The SDS signed in May 2016 did not disclose criminal history reflected on the Criminal Record Check (CRC), which was completed in June 2016. 2. Staff #5 - The SDS signed in June 2016 did not disclose criminal history reflected on the Criminal Record Check (CRC), which was completed in July 2016. Plan of Correction: Addressed Sworn Disclosure mistakes/violation with staff and counseled on proper instructions when completing SDS Forms. Both staff completed new SDS Forms correctly & immediately.
