22VAC40-191 Background Checks (22VAC40-191)

An unannounced monitoring inspection was conducted today, August 11, 2016 to determine the center's compliance with licensing standards. The inspection was initiated at 3:35pm and concluded at 5:40pm. Upon arrival, the following ratios were observed:

Room 1 (ages 6-8yrs): 2 staff/ 12 children
Room 2 (ages 4-5yrs): 1 staff/ 5 children
Room 3 (ages 8-12): 1 staff/ 8 children
Room 4 (ages 3yrs): 1 staff/ 5 children
Room 5 (ages 3-5yrs): 1 staff/ 5 children
Room 6 (ages 2yrs): 2 staff/ 5 children
Room 7 (ages 5-7yrs): 2 staff/ 10 children

Children were observed playing board games, coloring pictures, participating in free play, and interacting with teachers. Children's books, toys, and equipment were inspected and found to be age appropriate and in operable condition. All areas of the facility were inspected to include main hallways, classrooms and restrooms. The outdoor playground was inspected with the required resilient surfacing present and a new playground equipment installed.

The center had the following posted: license, daily schedule, emergency numbers, evacuation maps, food menu, previous inspection, and various parent information. These postings are located on the wall upon entering the center and in children's classrooms. The center's first aid kit and emergency supplies were inspected with additional supplies found. Ten children's and two staff records were inspected with the majority of the required information found captured; see attached notice. The following was also inspected: medication authorizations, annual health & fire inspections, evacuation drills, discipline policy, parent agreements, and other pertinent information.

Standard #: 22VAC40-185-60-A
Description: Based on the inspection of children's records, the required documentation was not present.

During the inspection of ten children's records, it was discovered child #3 did not have the physician's information listed in the record.

Plan of Correction: Per assistant director: I will contact the parent to get the required information and update the child's record.

Standard #: 22VAC40-185-270-F
Description: Based on the inspection of children's sleeping mats, the required condition was not maintained.

During the inspection of the three year old sleeping mats, 3-4 mats were observed to be torn and in unsatisfactory condition.

Plan of Correction: Corrected on site.

Standard #: 22VAC40-185-550-D
Description: Based on the inspection of administrative records the required information was not recorded. Evidence: During the inspection of the center's monthly evacuation drills, it was discovered the evacuation drill for July 2016 was not recorded. Evacuation drills are to be recorded each month with documentation maintained to review.

Plan of Correction: Per assistant director: We conducted a drill, however did not record it. We will ensure the drills are recorded as required.
