An unannounced monitoring inspection was conducted today from 8:45am-10:30am. There were 31 children directly supervised by 6 staff. The physical plant, 5 staff records, 5 children?s records, evacuation drills, and emergency supplies were inspected. Children were observed arriving to the center, participating in group play, singing songs, reading books, washing hands, and eating snack. There was an abundant supply of materials for the children. The center was clean and organized. Areas of non-compliance are identified in the Violation Notice. Standard #: 22VAC40-185-160-A Description: Based on record review, each staff member does not have documentation of a negative tuberculosis screening submitted within 21 days of hire. Evidence: Staff #3 (hire date: 7/26/15) does not have a negative tuberculosis screening submitted within 21 days of hire. Plan of Correction: I will ask the staff member to submit a TB screening as soon as possible.
Standard #: 22VAC40-185-60-A Description: Based on record review, the center did not maintain a record of two emergency contacts for all children in care. Evidence: Five children's files were reviewed, Child #3 is missing one emergency contact from the record. Plan of Correction: I will ask the parent and have them put the information in the file.
Standard #: 22VAC40-185-290-3 Description: Based on observation, the center did not have protective covers in all electrical outlets. Evidence: In the basement playroom, a total of 4 outlets in varying locations did not have protective covers installed. Two outlets by the exit door, one outlet behind a play structure on the right side of the room, and one outlet underneath the window to the left of the room were missing protective covers. Plan of Correction: We placed covers on the outlets on the day of the inspection.
