- Technical Assistance:
Technical assistance was given regarding the clear documentation of all food items offered by the center as it relates to the menu. Consultation was given regarding the measurements of food items as required by USDA served for the all age ranges.
An unannounced monitoring inspection was conducted on August 17, 2016 from approximately 9:50 a.m. until 2:15 p.m.. At the time of entrance there were 50 children and 10 staff members present with children. The sample size consisted of 5 children's records and 4 staff records. Two medications were reviewed. Children were observed playing in centers to include dramatic play, coloring and blocks, Children were also observed preparing for and eating lunch. Previous violations were reviewed. Areas of non-compliance are identified on the violation notice. The results of the inspection were reviewed and verified with the center director during the exit interview. Standard #: 22VAC40-185-160-A Description: Based on documentation review and staff interviews, the licensee did not ensure each staff member submitted documentation of a negative tuberculosis screening (TB). Documentation of the screening must be submitted no later than 21 days after employment and must have been completed within 12 months prior to employment. Evidence: During the inspection on August 17, 2016, the record for staff #4 (date of hire 3-2-2016) included a TB test was dated 1-23-2015 and is more than 12 months prior to the date of hire. Plan of Correction: The center director will have staff update TB.
Standard #: 22VAC40-185-60-A Description: Based on documentation review and staff interviews, the licensee did not ensure there was a record for each child enrolled with all required information. Evidence: During the inspection on August 17, 2016, the following records were determined to be incomplete.; 1) The record for child #1 did not include a home phone number for both parents listed. 2) The record for child #3 did not include the employment phone number for both parents listed. Plan of Correction: The center director will have records updated.
Standard #: 22VAC40-185-330-B Description: Based on observation, the licensee did not ensure where playground equipment is provided, there was an adequate amount of resilient surfacing under equipment with moving parts or climbing apparatus to create a fall zone free of hazardous obstacles. Fall zones are defined as the area underneath and surrounding equipment that requires a resilient surface. A fall zone shall encompass sufficient area to include the child's trajectory in the event of a fall while the equipment is in use. Evidence: During the inspection on August 17, 2016 the resilient surfacing on the preschool and school playgrounds were inadequate as it measures between 4 and 5 inches at the base of the green slides and around the yellow climbing bars. Plan of Correction: The surfacing will be replaced and the playground will be off limits starting today.
Standard #: 22VAC40-185-500-B Description: Based on observation, the licensee did not ensure that a nonabsorbent surface for diapering or changing was used for children younger than three years. Evidence: During the inspection on August 17, 2016, the diaper pads in classroom number 106 and 107 were determined to be absorbent as there were multiple tears in the diaper pad there was exposed foam. The diaper surface is required to be nonabsorbent. Plan of Correction: The center director will replace them.
Standard #: 22VAC40-185-530-A Description: Based on documentation review and staff interviews, the licensee did not ensure there was at least one staff member trained in first aid, cardiopulmonary resuscitation, and rescue breathing as appropriate to the age of the children in care who is on the premises during the center's hours of operation. Evidence: During the inspection on August 17, 2016 it was confirmed by center director that there was not one staff trained in first aid, CPR during the hours of 6:00 a.m. and 9:00 a.m. on August 17, 2016. Plan of Correction: The center director will change staff schedules until other staff are trained.
Standard #: 22VAC40-185-550-L Description: Based on observation and staff interviews, the licensee did not ensure the center notified the parent by the end of the day of any known minor injuries with all required information. Evidence; During the inspection on August 17, 2016, 3 injury reports were reviewed and all 3 did not contain the date, the time and how the parent was notified of the child's injury. Plan of Correction: The center director will retrain staff in filling out forms.
Standard #: 22VAC40-185-560-K Description: Based on observation, the licensee did not ensure tables were sanitized before and after each use for feeding. Evidence: During the inspection on August 17, 2016, the children in classroom 107 were observed preparing for lunch and the tables were not sanitized before they began lunch. Prior to the children eating lunch the licensing inspector observed children crawling on the tables and children sitting at the tables and playing. Plan of Correction: The center director will retrain staff today.
