Technical Assistance:
1. Reviewed precautionary measures to ensure the indoor air temperature is maintained in a healthy range. Once the temperature reaches 80 degrees fans or other cooling systems are to be used. 2. Rusting metal and bolts on the playground climbing structure should be monitored and removed.

Comments:
Conducted an unannounced monitoring inspection 11:40am-1:25pm. Observed 115 children + 15 direct-care staff. Ratios and supervision were in compliance. Children were having story time, eating lunch, and taking naps during the inspection. School-age children were playing in a video game trailer in the parking lot. Infants were being fed, having play time on the floor, and napping. The center's air conditioner was broken in the lobby, hallways, infant and toddler rooms where the air was hot, stuffy, and humid today. The indoor air-temperatures in these areas ranged from 81.4-83.5 degrees. Portable fans have been in use in these areas since Friday when the air-conditioner stopped working. Repairman were on-site during the inspection. There was a foul odor coming from the boy's bathroom. Efforts to improve the cleanliness of center noted, though long-term maintenance remains in-process. Resilient surfacing, wood mulch, was being replenished on the playground today. Standard #: 22VAC40-185-130-A
Description: Based on records reviewed, it was determined that 1 of 1 children's records did not include documentation of immunizations. Evidence: Child #1 was present today.

Plan of Correction: Recieved [sic] the paperwork from the parent at pick up

Standard #: 22VAC40-185-60-A
Description: Based on records reviewed, it was determined that 1 of 1 children's records did not include required enrollment information. Evidence: Child #1 - There was no documentation of the child's first date of attendance, date of birth, parent home/work information, authorized pick-up's, emergency contact information, physician and health information, a signed enrollment agreement, parental signature on ill-child and communicable disease policies, and emergency medication authorization.

Plan of Correction: Recieved [sic] paperwork from parent when they came to pick up.

Standard #: 22VAC40-185-240-D-4
Description: Based on records reviewed and interviews conducted, it was determined that there was no documentation of a staff person having completed medication administration training (MAT). Evidence: For approximately the first 30 minutes of the inspection there was no staff person present with documented MAT on-site, as needed for a child with emergency medications on-site.

Plan of Correction: Copy of valid Mat was mailed and received on 8/17/16

Standard #: 22VAC40-185-270-A
Description: Based on observations made, it was determined that the boy's bathroom was not maintained in a clean, safe, and operable condition. Evidence: One toilet was not working/not flushable and a strong foul odor was present in the bathroom. The smell was causing the bathroom and adjacent hallway to smell badly.

Plan of Correction: Work order has been placed on 7/28/16

Standard #: 22VAC40-185-290-3
Description: Based on observations made, it was determined that there were two electrical outlets in the lobby area that did not have protective covers.

Plan of Correction: Outlets covered during visit.
