22VAC40-191 Background Checks (22VAC40-191)
An unannounced monitor inspection was conducted on 8/23/2016. LI Anderson started inspection at 2:10 PM and concluded at 3:45 PM. There were a total of seventeen (17) children ages 4-10 years old present under the direct supervision of three (3) staff during the inspection. Children medication & staff records were reviewed. Children were observed during arrival/departure procedures, afternoon snacks, bathroom/handwashing procedures & independent activities. Risk Assessment & areas of non-compliance found during today?s inspection were reviewed with the Center?s Representative on 8/23/2016.
Standard #: 22VAC40-185-160-C Description: Based on record review, the Licensee failed to ensure each staff member updates tuberculosis skin test/screening every 2 years. EVIDENCE: TB skin test/screening for Staff #2 was not updated within 2 years since the most recent TB skin test/screening. Most recent TB test/screening was completed on 6/15/2013. Plan of Correction: Staff member will update TB skin test/screening. An appointment to complete the test has been schedule for this week.
Standard #: 22VAC40-185-280-B Description: Based on inspection, the Licensee failed to keep hazardous substances in a locked place using a sage locking method that prevents access by children. EVIDENCE: Concentrated cleaning agents was not stored in a locked location. Hazardous cleaning agent observed in the cafeteria area occupied by the children; unlocked & accessible to the children in care. Plan of Correction: Staff will inspect area daily to ensure hazardous agents are keep locked. Staff contacted school maintenance to remove cleaning agent.
Standard #: 22VAC40-185-290-3 Description: Based on observations, the Licensee failed to ensure all electrical outlets have protective covers. EVIDENCE: Electrical outlets were not covered with protective covers. LI Anderson observed several uncovered electrical outlets in the cafeteria area occupied by the children; accessible to the children. Pre-school children were present on the day of inspection. Plan of Correction: Staff will check outlets daily to make sure they are covered.
