During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Effective January 1, 2025, the VDOE will begin determining compliance with ? 22.1-289.057 of the Code of Virginia, which is legislation passed by the General Assembly in 2020. The law requires all licensed child day programs, religious exempt child day centers that serve preschool age children, and certified preschools to test potable drinking water. The law requires that programs submit their plans and test results to the Virginia Department of Health Office of Drinking Water (VDH ODW) and the Superintendent. If the results of the testing indicate elevated lead levels, the program shall remediate, retest, and resubmit results to VDH ODW and the Superintendent. There is an additional alternative bottled water option that comes with additional requirements. An announced, on-site initial inspection was initiated on January 5, 2026 and completed on January 5, 2026. The on-site inspection began at 10:00am and ended at 11:00am. The inspector reviewed compliance in the areas listed above. There were no children present and 1 caregiver. The inspector reviewed 3 caregiver/household member(s) records on-site.
This inspection included document review, tour of the facility, interviews, observations.
A subsidy health and safety inspection (SHSI) was conducted in conjunction with the licensing inspection. Subsidy standards that are not covered in licensing standards were reviewed during this SHSI.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on January 12, 2026 . A POC submitted after this date will not appear on the public website.

Standard #: 22.1-289.035-B-1 Description: Prior to hire, caregivers must provide a sworn statement or affirmation disclosing whether he has ever been convicted of or is the subject of pending charges for any offense within or outside the Commonwealth and whether he has been the subject of a founded complaint of child abuse or neglect within or outside the Commonwealth.
Caregiver 2's record did not have a sworn statement on file. Caregiver 2 has been employed for six months.Plan of Correction: Sworn statement has been filled and put into Caregiver?s file.
Standard #: 8VAC20-800-120-B Description: Assistants' and substitute providers' records shall contain the name address and telephone number of a person to be notified in an emergency.

Standard #: 8VAC20-800-320-G Description: Wood burning stoves and fireplaces and associated chimneys shall be inspected annually by a knowledgeable inspector to verify that the devices are properly installed, maintained, and cleaned as needed. Documentation of the inspection and cleaning shall be maintained by the provider.
There was no fireplace inspection documentation available for review during the inspection.Plan of Correction: An inspection for the upper-level chimney will take place.
