During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Comments:
An unannounced, on-site renewal inspection was initiated on 1/7/2026 and completed on 1/7/2026. The on-site inspection began at 9:50 am and ended at 11:10 am. The inspector reviewed compliance in the areas listed above. There were (10) children present and (3) caregivers; the point total was (36). The inspector reviewed (6) children?s records and (7) caregiver/household members records.

This inspection included document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-800-700-A
Description: Prescription medication may be administered if the parent has completed a written authorization from a parent and physician.
There was no written parent/guardian or physician authorization to administer prescription medications to child # 4.

Plan of Correction: The provider will provide the parent the medication authorization form to be signed by the parent and the physician.

Standard #: 8VAC20-800-70-A
Description: Before the child's first day of attendance, parents shall be provided required information in writing.'
The parents of Child #4 were not provided in writing the following required information: policy for administration of medications and provisions of the family day home?s emergency preparedness and response plan. Child had been in care for one month.

Plan of Correction: The parent will sign the documents and Provider will maintain in the child's file.
