Comments:
An unannounced monitoring inspection was conducted with the Senior Recreation Specialist beginning at 9:30 AM ending at 11:15 AM. There were eleven children in care ages ranging from three to five years old. The children were observed having breakfast which consisted of dry cereal with milk, goldfish, and fruit juice. Two staff records were reviewed. Four children's records were reviewed and determined to be complete. The fire & health inspections were reviewed. The last emergency escape drills were conducted on July 7 and July 28, 2016. Discussed with the Director: New Virginia Pre Service Training requirements. Standard #: 22VAC40-185-160-A
Description: Based on a review of two staff records not all of the records had documentation of a tuberculosis screening within 21 days of employment. Evidence: The record for staff A with a hire date of June 24, 2016 did not have documentation of a tuberculosis screening.

Plan of Correction: The facility will have the staff member get the tuberculosis screening by the end of this week.

Standard #: 22VAC40-185-160-C
Description: Based on a review of two staff records not all of the staff submitted tuberculosis screenings every two years. Evidence: The record Staff B with a hire date of December 1, 2009 did not have an updated tuberculosis screening.

Plan of Correction: The facility will not allow the staff to return to work until documentation of a tuberculosis screening is provided.

Description: Based on observation the facility did not obtain an annual fire inspection: Evidence: The last fire inspection documentation is dated September 26, 2014.

Plan of Correction: The facility will contact the Fire Marshal to obtain a copy of most recent inspection.

Standard #: 22VAC40-185-260-B
Description: Based on observation the facility did not obtain an annual health inspection: Evidence: The last health inspection documentation is dated October 20, 2014.

Plan of Correction: The facility will contact the health department to schedule a health inspection.

Standard #: 22VAC40-185-290-3
Description: Based on observation not all of the electrical outlets had protective covers. Evidence: Four of the electrical outlets did not have protective covers in the preschool classroom.

Plan of Correction: The facility will put protective covers into the electrical outlets.
