An unannounced monitoring inspection was conducted from 9:57 a.m. - 3:40 p.m. There were forty children in care with ten teachers working directly with them. There were two floaters to assist as needed. Three children's records were reviewed. The record for a new staff person was reviewed. The children were observed playing outside, working together with different sizes of tiles, building with colored boxes, playing beauty shop and pretend cooking, during story time, and washing their hands for lunch. The children are studying transportation and were working on an art project related to the many methods of transportation that exist. Areas of non-compliance are identified on the violation notice. Standard #: 22VAC40-185-130-B Description: Based on a record review the facility failed to have documentation of a physicians review for additional immunizations once every six months for a child in care. Evidence: 1. On 08/25/2016 the licensing inspector reviewed the record for child #2 and there was no documentation of a physicians review for immunizations once every six months. 2. On 08/25/2016 staff #1 confirmed there was no documentation of a six month review for immunizations by a physician in the record for child #2. Plan of Correction: We will request from the parent documentation of a review of immunizations by the physician for child #1. We should be able to have this review in the record in two weeks.
Standard #: 22VAC40-185-140-A Description: Based on record review there was no documentation of a physical for a child in care. Evidence: 1. On 08/25/2016 the licensing inspector reviewed the record for child #1 and there was no physical in the record. The child has been in care over 30 days. 2. On 08/25/2016 staff #1 reviewed the record for child #1 and confirmed there was no physical in the record. Staff #1 confirmed child #1 has been in care over 30 days. Plan of Correction: I will contact the parent today and we should have documentation of a physical for child #1 in the record within two weeks.
Standard #: 22VAC40-185-260-A Description: Based on a review of documentation the facility failed to have an annual fire inspection report for review. Evidence: 1. On 08/25/2016 the licensing inspector reviewed the most recent fire inspection which was dated 08/7/2015. 2. On 08/25/2016 staff #1 confirmed that the most recent fire inspection document was dated 08/07/2015. Plan of Correction: I will telephone the fire official and request this inspection as soon as possible. I will send the licensing inspector a copy of the fire inspector's report for the record.
Standard #: 22VAC40-185-510-A Description: Based on a review of medications the facility failed to prevent the usage of outdated medication. Evidence: 1. On 08/25/2016 the licensing inspector reviewed a medication for child #3. The medication had been filled by the pharmacy on 08/16/2016. The expiration date on the nebulizer packets inside the box was 04/2016. 2. On 08/25/2016 staff #1 confirmed the medication for child #3 had been filled by the pharmacy on 08/16/2016. Staff #1 also confirmed that the nebulizer packets inside the box had an expiration date of 04/2016. 3. On 08/25/2016 the licensing inspector reviewed the medication administration log. There was documentation that on 08/24/2016 staff #2 had administered the medication to child #3. Plan of Correction: The parent came and took the medication back to the pharmacy and they replaced the nebulizer packets with ones that expire 06/2017.
