During the inspection, the inspector reviewed the areas listed above. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Comments:
An unannounced, on-site renewal inspection was initiated and completed on 01/07/2026. The on-site inspection began at 1:00 p.m. and ended at 2:20 p.m. The inspector reviewed compliance in the areas listed above. There were three children present and one caregiver; the point total was eight. The inspector reviewed three children?s records and three caregiver/household members records on-site.

This inspection included:

A subsidy health and safety inspection (SHSI) was conducted in conjunction with the licensing inspection. Subsidy standards that are not covered in licensing standards were reviewed during this SHSI.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on 01/16/2026. A POC submitted after this date will not appear on the public website.

Violations:
Standard #: 8VAC20-770-40-D-5
Description: A household member 14 years of age and older must have a search of the central registry and make the information available for regulatory purposes.

Household member B (Age 14 years) did not have a completed central registry check on file for licensing review.

Plan of Correction: The provider has obtained a central registry check for household member B.

Standard #: 8VAC20-800-180-A
Description: A tuberculosis (TB) screening is required every two years from the last screening.

An updated TB screening for Household Member A was not completed and was overdue by three months.

Plan of Correction: Household Member A has obtained an updated TB screening.

Standard #: 8VAC20-800-240-A
Description: Areas and furnishings of the family day home, inside and outside, shall be maintained in a clean, safe, and operable condition. Unsafe conditions shall include, but not be limited to, the presence of poisonous plants; tripping hazards; unstable heavy equipment, furniture, or other items that a child could pull down on himself; splintered, cracked, or otherwise deteriorating wood; chipped or peeling paint; visible cracks, bending or warping, rusting, or breakage of any equipment; head entrapment hazards; and protruding nails, bolts, or other components that could entangle or could snag skin.

Five cots were observed stacked on top of a box located next to an occupied crib, creating a risk of being pulled down onto child B (age 2 years).

Plan of Correction: The provider has moved the cots away from the crib.

Standard #: 8VAC20-800-540-A
Description: When occupied, infant swings are required to be used according to the manufacturer's instructions and a safety strap shall be used and securely fastened.

Child A (Age: 6 months) was observed in a swing that was in motion without the safety strap securely fastened.
