During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

An unannounced, on-site renewal inspection was initiated and completed on 01/08/2026. The on-site inspection began at 11:00am and ended at 1:15pm. The inspector reviewed compliance in the areas listed above. There were four children present and one caregiver; the point total was 14. The inspector reviewed four children?s records and one caregiver?s records on-site. This inspection included document review, tour of the facility, interviews, and observations.
A subsidy health and safety inspection (SHSI) was conducted in conjunction with the licensing inspection. Subsidy standards that are not covered in licensing standards were reviewed during this SHSI.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard #: 8VAC20-790-250-F Description: Caregivers who work directly with children shall annually attend the department's health and safety update course. Caregiver #1 employed for over three years, did not have documentation of completion of the department's annual health and safety update course in 2025. Plan of Correction: I will complete the required training if I cannot locate documentation the it has been taken.
Standard #: 8VAC20-800-60-B Description: REPEAT VIOLATION
The provider shall maintain an up-to-date record at the family day home for each enrolled child. Child #1 did not have two required emergency contacts that include: name, address, city, zip code and phone number.Plan of Correction: I will request the required information and add it to the record.
Standard #: 8VAC20-800-90-C Description: Documentation of additional immunizations is required once every six months for children under the age of two years. 'Child #3, 21 months old, last documented immunization was over 10 months ago. Plan of Correction: I request the updated immunization records if I cannot locate them and it to the record.
Standard #: 8VAC20-800-180-A Description: A tuberculosis (TB) screening is required every two years from the last screening. An updated TB screening for Caregiver #1 was not completed and was overdue by six months. Plan of Correction: I will contact the doctor to obtain a copy of the updated TB screening or schedule an appointment to have one completed.
Standard #: 8VAC20-800-210-A Description: Caregivers shall obtain a minimum of 16 clock hours of training annually. Caregiver #1 could not located the annual training hours for 2025. Plan of Correction: I will search for the required training hours I have completed and take additional training if needed.
