Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Comments:

An unannounced, on-site monitoring inspection was initiated on 1/13/2026 and completed on 1/13/2026. The on-site inspection began at 9:20 am and ended at 10:15 am. The inspector reviewed compliance in the areas listed above. There were (6) children present and (3) caregivers. The inspector reviewed (6) children?s records and (5) caregiver/household member records. This inspection included: document review, tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program. A subsidy health and safety inspection (SHSI) was conducted in conjunction with the monitoring inspection. Subsidy standards that are not covered in voluntary registration standards were reviewed during this SHSI.

Standard #:	8VAC20-800-750-A
Description:	When topical skin products such as diaper ointment are used, the following requirements shall be met:, the product shall be labeled with the child's name. Child #2 and #5 diaper creams were not labeled with the child's name.
Plan of Correction:	All diaper creams will be labeled with the child's name.
Standard #:	8VAC20-800-750-B
Description:	Diaper ointment shall be inaccessible to children. Several diaper creams were found in the bathroom under the changing table accessible to children.
Plan of Correction:	All diaper creams will be stored out of the reach of children.
