Comments:
Today an unannounced monitoring inspection was conducted beginning at 10am. There were 115 children present with 19 teachers. Children were observed having storytime, preparing for a salt art science experiment, playing outside on the playground, and playing with manipulatives such as puzzles, blocks and legos. Nine staff records and 5 children's records were reviewed. Additionally, 8 medications and authorizations were reviewed. This inspection, including the exit interview, ended at 12:15pm. Violations:
Standard #: 22VAC40-185-70-A
Description: Based on record review, three staff records (#3, 4 and 9) are missing complete required documentation. Evidence: Staff #3 and 9 are missing documentation of a second completed reference check in their file. Staff #4 is missing documentation of their highest level of education qualifying them for their position.

Plan of Correction: We will get the second reference checks completed for these two staff. We will make sure this is done with new hires prior to their first day of employment. Will also ask staff #4 to provide their documentation of education.

Standard #: 22VAC40-185-260-A
Description: Based on discussion and record review, an annual fire inspection has not been conducted annually from the appropriate fire official. Evidence: The most recent annual fire inspection is dated 8/11/15.

Plan of Correction: We have received the paperwork to schedule the inspection. We will submit it immediately and annually before the current annual inspection expires.

Standard #: 22VAC40-185-510-N
Description: Based on a review of medications, four children's medications were not returned or disposed of by the center once the authorizations expired. Evidence: There were no current authorizations for medications present for 4 children (#7,8,9,10) per a discussion with the Director. The children are no longer taking the medications and the authorizations were purged from the medication binder, however, the medications were not returned to the parents or disposed of by the center.

Plan of Correction: We will tell parents today to take home the medications or we will dispose of them.

Standard #: 22VAC40-185-560-N
Description: Based on direct observation, three children were observed either eating or drinking while walking around. Evidence: In the Toddler A room one toddler was observed walking while drinking water from a sippy cup. In the Toddler B room one child was observed still chewing leftover snack while walking around and another toddler was observed drinking from a water bottle while walking around.

Plan of Correction: We will make sure children sit down until they are finished eating or drinking.
