Description: Based on record review, one newly hired staff member does not have a complete record. Evidence: Documentation that two references were checked prior to hire date (date of hire 8/1/2016).

Plan of Correction: References weer collected and placed in file.

Standard #: 22VAC40-185-270-A
Description: Based on observation, two areas of the side entrance are not being maintained in good and safe condition. Evidence: The door leading onto the playground is splintered in one window pane area and the garden box by the door is falling apart.

Plan of Correction: Garden box was removed. Window to be replaced.

Standard #: 22VAC40-185-550-M
Description: Based on record review, 2 of 10 injury reports checked were incomplete. Evidence: 1. 2 reports were missing how the parent was notified. 2. 2 reports did not list the time the parent was notified.

Plan of Correction: New document was generated and staff asked to list both notification method and time.
