Documentation of injury review should be completed annually. Time the parent is notified needs to be a time rather than the phrase "at pick-up"
An unannounced renewal study was conducted today. 17 children were present with 2 volunteers and 4 staff. Children were engaged in clay, coloring clowns, free play and transition to rest room. The lunch boxes were dated and labeled with each child's name-snack/lunch is bought from home. Emergency preparation is current for first aid/CPR and MAT certification. No medication was reported to be on-site during the study. 5 children's records were reviewed and complete. A sample of staff files for all Tot Camps was conducted today with the Program Director at Lubber Run following the renewal study. Five staff files were reviewed and found to be complete. Standard #: 22VAC40-185-270-A Description: Based on observation, an area of the playground is not being maintained in a safe and operable condition. Evidence: The bench that is built around the tree on the playground is chipping and peeling paint. Plan of Correction: Request sent to PNR to paint bench. Bench will be off limits until repainted and sanded.
Standard #: 22VAC40-185-550-E Description: Based on observation, an evacuation drill was not documented on the center record. Evidence: The drill for the month of July was not written down on the center evacuation log. Plan of Correction: The drill documentation was completed during the inspection.
Standard #: 22VAC40-185-550-M Description: Based on record review, minor injury forms are incomplete. Evidence: A random sample of injury logs found that all information was not documented as required. 1. 12 forms were missing future action to prevent reoccurrence of injury and a second signature. 2. Pick up time was missing from 1 form Plan of Correction: Will request line added to incident reports. Approval requested from risk management. Specific pick up time will be added.
