During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Comments:
An unannounced, on-site monitoring inspection was initiated on 2/3/26 and completed on the same day, as a part of licensure period. The on-site inspection began at 12:15 and ended at 13:25. The inspector reviewed compliance in the areas listed above. There were 12 children present and 2 caregivers at the beginning of the inspection. The provider arrived 20 minutes after the inspection started..The point total was 30. The inspector reviewed 4 children?s records and 3 caregiver/household members records on-site.
This inspection included: document review, tour of the facility, interviews, and observations.
A subsidy health and safety inspection (SHSI) was conducted in conjunction with the licensing inspection. Subsidy standards that are not covered in licensing standards were reviewed during this SHSI.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Violations:
Standard #: 22.1-289.057-A
Description: REPEAT VIOLATION
Each child day program that is licensed pursuant to this chapter and any program described in subdivision A 4, B 1, or B 5 of ? 22.1-289.030 that serves preschool-age children shall develop and implement a plan to test potable water from sources identified by the U.S. Environmental Protection Agency as high priority, including bubbler-style and cooler-style drinking fountains, kitchen taps, classroom combination sinks and drinking fountains, home economics room sinks, teacher's lounge sinks, nurse's office sinks, classroom sinks in special education classrooms, and sinks known to be or visibly used for consumption.

As of January 2025 the provider had to have the water tested for lead. The provider has not had the testing done yet.

Plan of Correction: I have tried to get the lead testing done but it has been very confusing for me. I have a link that I will try to use.

Standard #: 8VAC20-800-60-A
Description: The provider shall maintain an up-to-date record at the family day home for each enrolled child.

One child in care does not have a file, with information required.

Plan of Correction: We have the brother of the child in care and the parent said they will have the paperwork to us end of this week.
