A complaint was received on 8/2/16 from local DSS agency alleging a child in care had fractured writs and arm from a fall at the center. There were other concerns expressed about supervision of a special needs child on the playground. An unannounced inspection was made on 8/4/16 with a local DSS CPS investigator. Three staff were interviewed and the child was observed on the playground. Collateral interviews were done by the CPS investigator and shared with the inspector. The inspection was completed and sent to the provider on 9/9/16. The interviews and observations were made between 10:00 am and 5:30 pm. Based on interviews with staff, interviews with collateral parties, observation of the child, and review of staff and child records, the complaint was determined not valid for lack of supervision.
