During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Technical Assistance:
* Discussion that an updated "Instructions for Completing Background Checks" can be found on the background check webpage of the public website. Key changes include: removal of managers' direct contact information, transition to providers paying for fingerprints, fee increases for fingerprints/central registry/portability, Fieldprint registration process for providers and applicants for fingerprints, unable to determine will no longer be used as a final decision - only eligible or not eligible will be used. It is now a longer document (21 pages) to give providers more detail in completing the required steps to transition to paying for background checks. There are also some other minor changes. All Providers are encouraged to use the portal for online submission of central registry requests.

* Beginning February 1, 2026, licensed child day programs, licensed family day systems, and voluntarily registered family day homes shall refer to Chapter 8VAC20-821, General Procedures for the Licensure of Child Day Programs and Family Day Systems and Background Checks. This chapter replaces 8VAC20-770, Background Checks for Child Day Programs and Family Day Systems; 8VAC20-820, General Procedures and Information for Licensure; and 8VAC20-830, Fee Requirements for Processing Applications. * Information about ?Pass? for Child Care Subsidy Providers ?

Child Care Program Attendance and Schedule System (PASS) is the new attendance tracking platform for the Subsidy Program. Child Care Subsidy Providers will no longer record a hard copy attendance (last day 11/30/2025). No more swipe cards or POS machines. Attendance will be recorded on tablets starting 12/01/25.

Comments:
An unannounced, on-site renewal inspection was initiated and completed on 02/03/26. The on-site inspection began at 10 am and ended at 11:30 am. The inspector reviewed compliance in the areas listed above. There were (5) children in care with (2) resident children present and (2) caregivers; the point total was (16). The inspector reviewed (5) children?s records, (2) caregiver records, and (3) household member records on-site on 02/03/2026.

This inspection included: document review (i.e. Files, injury logs, policies and procedures, emergency drill logs), tour of the facility, interviews, and observations.

A subsidy health and safety inspection (SHSI) was conducted in conjunction with the licensing inspection. Subsidy standards that are not covered in licensing standards were reviewed during this SHSI.

Information gathered during the inspection determined non-compliance with (3) three applicable standards. These violations are documented on the violation notice issued to the home.

Violations:
Standard #: 8VAC20-800-780-B
Description: Annually, the provider shall review with the parent the emergency contact information required in each child?s record to ensure the information is correct and shall obtain the parents signed acknowledgment of the review.

Records for children #1, #2, and #3 did not have a parent?s signature for an annual review of the emergency contact information. All three had been in care over the past year without an annual review.

Plan of Correction: Reviews will be conducted and signed by the parent as verification.

Standard #: 8VAC20-800-90-A
Description: Before attendance, documentation of immunizations is to be obtained for each child.

There was no documentation of immunizations for Child #5, who was in care during the inspection and had been in care for approximately (4) four months.

Plan of Correction: An immunization page for Child #5 will be obtain from the child's parent.

Standard #: 8VAC20-800-90-C
Description: Documentation of additional immunizations is required once every six months for children under the age of two years. '

Child #4, 11 months old, last documented immunization was over 7 months ago.

Plan of Correction: An updated immunization page for Child #4 will be obtain from the child's parent.
