An unannounced, on-site monitoring inspection was initiated on 02/10/2026 and completed on 02/10/2026. The on-site inspection began at 10:05 am and ended at 11:45 am. The inspector reviewed compliance in the areas listed above. There were 9 children present and 3 caregivers; the point total was 24. The inspector reviewed 9 children's records and 3 caregiver records on-site on 02/10/2026. This inspection included:

A subsidy health and safety inspection (SHSI) was conducted in conjunction with the licensing inspection. Subsidy standards that are not covered in licensing standards were reviewed during this SHSI, and this single inspection report reflects findings related to licensing standards and subsidy standards.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard #: 8VAC20-790-170-G Description: The vendor shall maintain, in a way that is accessible to all caregivers, a current written list of all children's allergies, sensitivities, and dietary restrictions. This list shall be dated and kept confidential.
The provider did not have a current written list listing food sensitivities and dietary restrictions for child 4 and child 5.Plan of Correction: The food sensitivity / dietary restriction list was created after the inspection.
Standard #: 8VAC20-790-250-F Description: Caregivers who work directly with children shall annually complete the department's health and safety update course.
Caregiver 1 and caregiver 2 did not complete the 3-hour annual subsidy health and safety course for 2025.Plan of Correction: The provider stated that both caregivers will begin the 3 hour training for 2026 immediately.
Standard #: 8VAC20-800-890 Description: When food is brought from home, the following shall apply: The food container shall be clearly labeled in a way that identifies the owner.
One container of frozen breast milk, for child 1, was not labeled with the child's name.Plan of Correction: The provider stated that all frozen breast milk will be checked for a name label going forward.
Standard #: 8VAC20-800-960-E Description: Bottles shall be refrigerated and labeled with the child's full name and the date, if more than one infant is in care.
A bottle containing breast milk, for child 2, was not labeled with the child's full name and date.Plan of Correction: The provider stated that the bottle had been labeled but the name had washed off. The bottle was re-labeled with the child's name during the inspection.
Standard #: 8VAC20-800-100-A Description: The provider shall obtain documentation of a physical examination by or under the direction of a physician prior to a child's attendance or within 30 days after the first day of attendance.
There was no documentation of a physical exam on file for child 3. Child 3 has been in care for more than 30 days.Plan of Correction: The provider stated that a physical exam had been requested several times. The parent will be asked to provide this documentation as soon as possible.
Standard #: 8VAC20-800-90-A Description: Before a child may attend the family day home, the provider shall obtain documentation that the child has been adequately immunized according to the requirements of ? 32.1-46 A of the Code of Virginia and applicable State Board of Health regulations.
The immunization record for child 3 did not provide documentation that child 3 was adequately immunized according to his age.Plan of Correction: The parent provided the immunization documentation later in the afternoon on the date of the inspection.
