22VAC40-185 STAFFING AND SUPERVISION.
An unannounced other inspection was conducted with center director and center staff. The self reported situation regarding the PREP classroom and incident reports was validated. Standard #: 22VAC40-185-340-A Description: Based on observation and review of incident reports, the facility failed to ensure the care, protection, and guidance of children in care. Evidence: from 11/2015 to 5/2016, the PREP classroom had documentation of 34 incident reports for Child A and 14 incidents reports documented for Child B. "Incidents" included the children pushing, biting, hitting, and throwing objects. During today's inspection, Child A became frustrated and threw a toy and the classroom staff did not redirect the child. The director intervened to redirect the child's attention. Plan of Correction: Staff will be retrained on positive guidance no later than October 31, 2016. Teachers will keep a log of biting incidents in the classroom in order to identify any repetitive patterns and develop plans to change the negative behaviors. The center will continue using the behavior management plans as well as having parent meetings in order to ensure effective communication between the center and families. Neither Child A nor Child B are currently enrolled at the center.
