Conducted an unannounced monitoring study with the Director, one staff, and eleven children. The ages of the children range from five years-old through nine years-old. The children were having free play in the cafeteria during the inspection. Two staff records and five children's records were reviewed. There were no children on medication today. Start time: 6:00am End time: 7:10am
Standard #: 22VAC40-185-130-A Description: Based on a review of five children's records, one record was missing the immunization record. Evidence: Child A did not have any documentation of immunizations in the record, and this must be in the child's record before a child can attend the center. Plan of Correction: The site director will get the missing information from the parent by today.
Standard #: 22VAC40-185-260-A Description: Based on direct observation, there was not a fire inspection. Evidence: There was not any documentation of an annual fire inspection. Plan of Correction: The site director will get the documentation from school personnel.
